berce c228df205e fix(install): name the symlink target when writing CLAUDE.md (#3805) ...

When ~/.claude/CLAUDE.md is a symlink, the registration block is written
into the file it points to, but install only printed the link path, so a
rules file elsewhere changed without any mention. Print `link -> target`
for symlinks on install and on the project-scoped removal path.
