* FetchStatusCodes
    add 404 constant
* ExtractorParameters
    rename isIndependentExtractor (which seems to describe the parameters object) to getExtractIndependently (the paramters' imperative to extractors)
    add getExtract404s
* Extractor
    add default ExtractorParameters impl
    remove redundant shouldProcess
* ContentExtractor
    consult getExtract404s, if false skip extraction of 404s
