Landing on /app/<name> (or choosing an option in the add-app modal) adds the app's tile before the launch runs, because the launch is what installs it. When the launch failed — no access, no app token, a thrown error — the tile stayed for the rest of the session, survived refreshes, and every click on it failed again. The launch outcome is now passed to settleDeepLinkLaunch, which takes a tile it drew back out (and restores a previous uninstall record) when nothing was launched.
