fix(skill): aider and devin code-only runs reach the merge (#4447)

On aider and devin nothing wrote .graphify_semantic.json for a code-only
run, so Part C raised FileNotFoundError, and the --update paragraph sent
the agent from Part A to a merge that reads .graphify_extract.json.

Part B now has the fast-path block, the paragraph names Step 3A, the
fast path, Part C and the merge, and aider's only-deletions guard writes
the file its merge reads.

Adds tests/test_code_only_update_all_hosts.py for the two monoliths and
a skillgen audit entry for the new lines; the 14 generated hosts are
left to #4313.

"""#4447: a code-only ``--update`` must reach the merge with this run's AST in it.
The ``--update`` runbook says what the agent does when ``code_only`` is True. This test
reads that paragraph from the RENDERED skill, runs the blocks the paragraph names (Part A,
plus the Part B fast path and Part C only if it names them), then runs the merge and checks
what came out. The route is not hard-coded: a paragraph that names too little fails here,
which is the bug (Part A writes only ``.graphify_ast.json``; the merge reads
``.graphify_extract.json``).
Hosts: the 14 generated skills share one set of anchors (windows runs its PowerShell
here-string bodies through the interpreter on stdin: it tests the Python and the step order,
not PowerShell quoting). aider and devin are monoliths with their own anchors; neither update
section writes ``.graphify_detect.json``, so the test seeds it for them.

If `code_only` is True: print `[graphify update] Code-only changes detected - skipping semantic extraction (no LLM needed)`, run Step 3A (AST) on the changed files, then run the Part B fast path to write an empty `.graphify_semantic.json`, then run Part C, which builds `.graphify_extract.json` from the new AST result. Do not dispatch subagents or read the semantic cache. Then go on to the merge and Steps 4-8.
If `code_only` is False (any changed file is a doc/paper/image): run the full Steps 3A-3C pipeline as normal.

In the two monoliths the Part B fast path was prose only, so Part C's
unconditional read of ``.graphify_semantic.json`` raised FileNotFoundError on
a code-only build or ``--update``, and the code-only paragraph never named
Part C. The fast path now writes an empty semantic file first, the paragraph
names Step 3A, the fast path, Part C and then the merge, and aider's
only-deletions guard writes the ``.graphify_extract.json`` its merge reads.

any single copy of those lines); the rest are the old and new lines.
