safishamsiandClaude Opus 4.8 e72af53c37 fix(path): fail closed on a file::symbol endpoint the file does not define (#4264) ...

A file-scoped path endpoint that matched no symbol in that file fell through to
bag-of-words scoring, which then answered from a same-named symbol in another
file. Refuse like explain does (listing the files that do define the symbol)
instead of guessing across files; fixes the CLI path and the MCP shortest_path
tool via the shared resolver. Native Rust/C++ :: labels resolve via the exact
tier and are unaffected.
