Change fixed clang-format version to 15
...
* We commit statically linked x64 binaries for clang-format-15 for windows and
linux to make the transition easier for anyone running a linux distro that
doesn't yet support llvm 15, or in future for distros that drop support for
it.

Update build requirements for Apple
...
CMake version 3.20 or higher (needed to support Xcode project generation)
clang version 12.0 or higher (needed to support arm64+x64 universal binary compilation)
XCode 12.2 is the oldest version which includes clang version 12.0

Add guidance on filing good issues, and update issue template
...
* Issues not following the template will be closed. It is too much of a drain on
project resources to deal with bug reports that are not well formatted and
unclear. The template is easy to fill out and does not ask much.
